That’s when the Watchmen (mutated dog-like creatures) attack you. At some point you will have to push another railcar out of the way. The last tricky one can be found when taking the stairs down the roof, in a little side room. The next one is a woman in the shower in a house on the left after the car accident vision. The first tricky one is after the playground vision, after you come out of the underground area in a house to your right (before the car accident vision). So only three of them are a little tricky to find. Four of the visions are triggered automatically and there is no way to miss them. Everything gets bright colors and the environment changes to what it once was. These visions make the world around you feel more alive for a few seconds. This is story related and can only be done during the “The Dead City” chapter. There are lots of other crates and ammo locations in the swamps, but only the shown 16 do count for this trophy / achievement.
